Terms and conditions for Unlimited Monthly Membership

For Unlimited classes per month including any combination of Stroller Strides, Stroller Barre, Strides 360, and Body Ignite classes in the Fort Worth franchise.

Unused visits cannot roll over to the following month.

· $59 Registration Fee

· Non-Refundable

· Non-Transferable

· Monthly fee of $79 will be charged to your credit card automatically every month.

· Autopay will continue until you cancel your membership.

· 14 days written notice to cancel membership.

· Client must commit to 4 months of membership payments. Early cancellation will result in a fee of $150.

AUTHORIZATION FOR PAYMENT
I authorize Fort Worth FIT4MOM (Trinity Trails Fitness, LLC) to initiate a charge to the account entered into Front Desk for my dues. This authorization is to remain effective until I have canceled my membership through the Fort Worth FIT4MOM administrator.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with Fort Worth FIT4MOM to fulfill my payment commitment and I am obligated to pay. Dues will be drawn monthly from the account on the date that the membership began. The processing date for debit/credit cards may vary due to banking procedures. If charges are returned they will be subject to a late fee. Members are responsible for updating their credit card information online. Members who have unpaid balances may not attend classes. If a membership remains unpaid it will be canceled with cancellation fees applied.

STROLLER POLICY
Children must be completely buckled into their strollers at all times. This is for their safety. Children under 2 months of age must be buckled safely in car seats in their strollers. Running with infants younger than 6 months in a stroller is not recommended. Please check your stroller brand guidelines. If a child is attending class with a friend or babysitter and their parent is not present, their parent or legal guardian must sign a waiver beforehand.

CANCELLATION POLICY
To cancel this membership, a written notice to fortworth@fit4mom.com is required 14 days prior to the cancellation date. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Fort Worth franchise. Once a membership is canceled, if the member wants to re-enroll they will be charged a $50 registration fee and will re-enroll at the new membership price.

HOLD POLICY
To place a membership on hold, a written notice to fortworth@fit4mom.com is required 14 days prior to the beginning hold date. The written notice must include a start date (at least two weeks after the date of the letter sent) and an end date. Memberships may be on hold a maximum of 3 months per calendar year. There will be a one-time $10 processing fee charged to your credit card on file per hold period. When a hold period has ended, the monthly auto payments will resume on the return date. No refunds will be given for dues once it has been paid.
